What is AI Nurse Public Link?

AI Nurse Public Link lets you create automated intake forms that collect health information through an AI-powered chatbot. The bot asks questions conversationally and adapts based on patient responses. 

How it works: Generate a unique link for each form type (initial intake, follow-up, specialty consultation). Embed the link in your booking confirmations, website, or emails. When patients complete the form, their chart appears automatically in your system.

 

You can also collect consent forms. The AI Nurse now fully automates patient consent collection, seamlessly combining legal consent forms with the clinical intake flow.


Step 1: Create Your Sequence Template

The Sequence Template is your list of questions and instructions for the AI chatbot.

Click Create Public New Link, then click the green Create New Sequence Template button.

Name your template clearly for easy reuse:

  • Initial Visit
  • Follow-Up
  • Menopause Consultation
  • Pre-Surgical Assessment

Sequence Instructions

These are the questions your chatbot will ask. 
You have two demo templates to start with:

  • Pre-Charting Template (recommended starting point)
  • GAD-7 Template

đź’ˇ Tip: Click on the Pre-Charting template and edit it to match your specialty and workflow. The AI will ask questions one at a time and adapt based on patient responses.

This welcome message appears immediately when patients open the form. Customize it to reflect your clinic's voice:

Compliance Settings

Add required consent and privacy policies. You can edit the default privacy policy by entering:

  • Organization Name
  • Provider Name

The system includes a standard privacy policy template. Customize it to match your legal requirements and jurisdiction.

Choose whether patients interact via voice or text chat.

Routine Completion Detection

Receive an email notification when patients complete the intake. To enable this:

To enable this:

  1. Enter a closing phrase here in the Routine Completion Detection field. 
    Example: "Thank you for completing the intake. We look forward to seeing you at your appointment. You can now close this tab."
  2. Go back to Page 1: Basics and add that same exact phrase at the very end of your Sequence Instructions (after all your questions)
  3. When the AI says this phrase to the patient, you'll automatically receive an email notification that the intake is complete

Click Create Template when finished.

Step 2: Configure Your Public Link

Now that your template is ready, set up the shareable link.

Event Name

This appears to patients when they open the link:

  • New Patient Intake, Follow-Up Session, Annual Physical

Event Description

Optional brief description of what the intake covers.

Appointment Booking URL

Add your scheduling link if patients need to book or reschedule appointments.

Session Limits

  • Hourly Session Limit: Maximum number of different patients who can fill out the intake per hour (default: 10). Increase for high-volume practices.
  • Daily Session Limit: Maximum number of different patients who can fill out the intake per day.

Practice Logo

Upload your logo to brand the patient experience.

Click Create Link.

Step 3: Share Your Link

Your permanent public link is now ready. Copy it and share via:

  • Appointment confirmation emails
  • Booking website integration
  • Text message reminders
  • Patient portal

Link Management: You can deactivate or delete links anytime. Each link includes built-in reCAPTCHA protection against bots.

Step 4: Monitor & Analytics

Click on your link to view its analytics page:

  • Recent Access Logs: See when patients accessed the form
  • Email Verification Status: Track verified vs unverified entries
  • Completion Rate: Monitor how many patients finish the intake
  • Bot Protection: View reCAPTCHA activity

Step 5: Review Completed Sessions

Below your link, you'll see all completed AI Nurse sessions:


 

Status: In Process or Completed

Actions: View Responses and Transfer Session

  • Create New Patient Record: Automatically creates a new patient record from the intake
  • Transfer to Existing Patient: Merge with an existing patient chart

Auto-Generated Charts: The system automatically creates SOAP notes, intakes, questionnaires (GAD-7, PHQ-9), and patient records from the intake data. The chart appears in your system ready for review.

Accessing Legacy AI Nurse Copilot 1.0
