  • Open AI Agents and select AI Nurse Assistant
  • Create your own Intake Questionnaire
  • Send via Email, SMS, or use in the clinic
  • Track status
  • Transfer response to the patient record

Step 1: Launch AI Nurse Assistant

In the left sidebar:

Click AI Agents > AI Nurse Assistant

Open AI Nurse Assistant

Step 2: Create a Nurse Template

Click Create AI Nurse Copilot 

Under Select AI Routine Template click Create AI Nurse CoPilot Sequence Template

Step 3: What is a Nurse Template

Your nurse template is the list of questions which AI Nurse Assistant asks your patients.

🧩 Basic Nurse Template Example

You are an AI medical assistant that helps the doctor with pre-charting. Follow these rules and ask one question at a time.

Ask the patient:
1. Please provide your full name.
2. What is your date of birth?
3. What is today's date?
4. What is the primary reason for seeking medical care today? Please describe your main concerns in a few words.
5. Can you describe the history of your present illness, including onset, duration, location, quality, severity, timing, context, modifying factors, and associated signs or symptoms?

Tip: Customize this list with your specialty prompts as needed.

Step 4: Enhance your Patient Experience

  • Sequence Title: a clear name for quick reuse
  • Sequence Instructions: your question flow

Click Show Advanced Settings to add optionally:

  • Custom Welcome Message: a short greeting that explains the intake process
  • Compliance Settings: attach consent or terms if your clinic requires them
  • Email Subject & Content: the message your patient receives.
  • Interaction Mode: Voice or Text
  • Routine Completion Detection (recommended): Tracks when a patient finishes the questionnaire.

How to set up the "Finished" notification?

Inside your main Nurse Template, include a short closing message to the patient. Common examples:

  • “Thank you for filling out the form.”

  • “Your intake is complete. If you have urgent concerns, please call our clinic at [phone number].”

⚡ Important: This phrase must appear twice:

  1. Inside your Sequence Instructions.

  2. In the Routine Completion Detection field.

When the AI Nurse Assistant delivers this phrase, the system recognizes that the intake is finished, changes the status to Completed, and triggers your notification.

Step 5: Submit & Save

Step 6: Send to Patient

Enter the patient's name, then choose how to send the form:

  • Email
  • SMS
  • Clinic Mode: best when the patient is on site to streamline triage

 You can also add your clinic logo and select the interaction language.

Step 7: Monitor Progress

Use the AI Nurse Assistant list to track status:

  • In Progress
  • Completed (triggered by your completion phrase)

Step 8: Transfer Results

When a form is completed, click the patient’s name to see their response:

You can also transfer it to their main thread

  • Transfer to Patient Thread for new patients
  • Transfer to a Specific Patient to attach to an existing chart

Your nurse template is live. You can now capture pre-charting information smoothly and speed up your clinical workflow.
