Automated Patient Intake
The AI Nurse is a patient-facing agent that collects pre-visit information through shareable public links. Navigate to AI Nurse in the sidebar to access this feature. The AI Nurse conducts conversational intake sessions with patients, gathering medical history, symptoms, and consent — all before the patient arrives at your office.
How it works: Generate a unique link for each form type (initial intake, follow-up, specialty consultation). Embed the link in your booking confirmations, website, or emails. When patients complete the form, their chart appears automatically in your system.
How the Patient Experience Works
- You share the intake link — copy it to your website, email it directly, or have the AI Receptionist text it during a call
- Patient clicks the link — no login or account creation required
- Patient verifies via SMS — they enter their name, email, and phone, then receive a 6-digit code to verify their identity
- Patient completes intake — consent forms, insurance card upload, and AI-guided questionnaire
- You review the results in your DeepCura dashboard and sync to your EHR if connected
Creating an Intake Template
Before generating a link, you need an intake template. Click Create Template to open the template editor, which walks you through a 3-step wizard:
Page 1: Basics
- Sequence Title — Name your template clearly for easy reuse. Examples: Initial Visit, Follow-Up, Menopause Consultation, Pre-Surgical Assessment.

Sequence Instructions — These are the questions your AI Nurse will ask patients, one at a time. Two demo templates are included to get you started:
- Pre-Charting Template (recommended starting point)
- GAD-7 Template
- Email Notification — Configure the email notification sent to you when a patient completes their intake session.
Sample Nurse Template
You are an AI medical assistant that helps the doctor with pre-charting. Follow these rules and ask one question at a time.
Ask the patient:
1. Please provide your full name.
2. What is your date of birth?
3. What is today's date?
4. What is the primary reason for seeking medical care today?
5. Can you describe the history of your present illness, including onset, duration, location, quality, severity, timing, context, modifying factors, and associated signs or symptoms?

Tip: Customize this list with your specialty prompts as needed.
Tip: Click on the Pre-Charting template and edit it to match your specialty and workflow. The AI will ask questions one at a time and adapt based on patient responses.
Page 2: Communication Settings
- AI Nurse's First Message — This welcome message appears immediately when patients open the form. Customize it to reflect your clinic's voice.
- Compliance Settings — Add required consent and privacy policies. Customize the default privacy policy with your Organization Name and Provider Name to match your legal requirements and jurisdiction.
Page 3: Advanced Options
- Voice Configuration — Toggle voice interaction on or off so patients can speak their responses instead of typing.
- Routine Completion Detection — Set a completion detection keyword that signals when the intake routine is finished and triggers an email notification to you.
How to set up the completion notification:
- Enter a closing phrase in the Routine Completion Detection field.
Example: "Thank you for completing the intake. We look forward to seeing you at your appointment. You can now close this tab." - Go back to Page 1: Basics and add that same exact phrase at the very end of your Sequence Instructions (after all your questions).
- When the AI says this phrase to the patient, you'll automatically receive an email notification that the intake is complete.
Generating a Public Link
Once your template is ready, create a public link by clicking Create Link. Each link is configured with:
- Template — The intake template the AI Nurse will follow.
- Display Name — The practice or provider name shown to patients (e.g., "New Patient Intake", "Annual Physical", "Follow-Up Session").
- Event Description — Optional brief description of what the intake covers.
- Logo — Upload a custom logo that appears on the intake page for branding.
- Session Limits — Set daily and hourly session limits to prevent abuse (defaults: 50 daily, 10 hourly). Increase limits for high-volume practices.
- Calendar URL — Optionally link a Calendly or booking page for patients to schedule after intake.
- Consent Forms — Attach consent forms from your library that patients must acknowledge during the session.
The generated link can be shared via email, text message, website embed, or QR code. Patients click the link, interact with the AI Nurse, and their responses are recorded in DeepCura.
Link Management: You can deactivate or delete links anytime. Each link includes built-in reCAPTCHA protection against bots.
What Patients Can Do During Intake
Sign Consent Forms Digitally
Upload your practice's consent PDFs (HIPAA authorization, financial policies, cancellation policies, etc.) and attach them to your intake link. Patients view each form and sign digitally on their phone or computer.
- Signature capture works on all devices (phone, tablet, desktop)
- Patient name, date, provider name, and organization name auto-fill into form fields
- Signed PDFs are saved securely in your account
- A system HIPAA consent form is automatically included
Upload Insurance Card Photos
Patients can photograph their insurance card and upload it during intake. DeepCura uses AI vision to automatically extract:
- Insurance provider name (e.g., Aetna, Blue Cross Blue Shield)
- Policy/member ID number
- Group number
- Coverage details and plan type
Extracted data pre-fills the patient's record — no manual data entry needed.
Answer Pre-Visit Questions
After verification and consent, patients enter a conversational AI session. The AI Nurse asks dynamic, follow-up questions based on the visit type and the patient's responses — going beyond static forms to capture clinically relevant details.
- Questions adapt based on patient answers (not a fixed checklist)
- Conversation is recorded and stored as a transcript in your dashboard
- Supports multiple languages
Monitor & Analytics
Click on any active link to view its analytics page:
- Recent Access Logs — See when patients accessed the form
- Email Verification Status — Track verified vs unverified entries
- Completion Rate — Monitor how many patients finish the intake
- Bot Protection — View reCAPTCHA activity
Reviewing Intake Sessions
Completed sessions appear in a table below your active links. Each session shows the patient name, completion status, and timestamp. You can click a session to view the full conversation transcript.
A Transfer feature lets you move intake data to an existing patient record in your directory. This links the intake session to the patient, making the pre-visit information available alongside their notes and transcripts.
- Create New Patient Record — Automatically creates a new patient record from the intake
- Transfer to Existing Patient — Merge with an existing patient chart
Auto-Generated Charts: The system automatically creates SOAP notes, intakes, questionnaires (GAD-7, PHQ-9), and patient records from the intake data. The chart appears in your system ready for review.
Consent Form Management
The AI Nurse integrates with DeepCura's Consent Form Library. You can create, manage, and attach consent forms to intake links. When a patient completes a consent form during their AI Nurse session, a signed PDF is generated and stored in the patient's record.
EHR Integration
Intake data flows into your DeepCura dashboard as a patient note. From there, what gets synced to your EHR depends on the data type and your connected system.
What Syncs to Your EHR
If you have an EHR connected (eClinicalWorks, Athena, Epic, AdvancedMD, or CharmHealth), the following clinical data can be pushed directly into the patient's chart:
- Clinical notes — the finalized encounter note generated from intake and AI Nurse conversations
- Conditions and diagnoses — ICD-10 and SNOMED-coded problems written to the patient's problem list
- Allergies — SNOMED CT-coded allergens with criticality levels
- Medications — RxNorm-coded prescriptions and medication reconciliation
- Lab and imaging orders — LOINC-coded service requests with clinical justification
- Immunizations — CVX-coded vaccine records with site, route, dose, and lot tracking
What Stays in DeepCura
- Signed consent PDFs — viewable and downloadable from the patient note
- Insurance card photos and extracted data — stored with the patient record for reference
- Raw AI Nurse conversation transcripts — the full question-and-answer exchange
- Custom form responses — completed intake form data from your uploaded PDF forms
Security & Compliance
- SMS verification — 6-digit OTP code with 15-minute expiry prevents unauthorized access
- No patient account required — phone number is the only identifier needed
