Fill Structured Forms with your Voice

📍 Navigate to: Open any patient note → Forms button in toolbar · Full UI Guide


You can now create Quick Phrases that are entire forms, like a treatment report or patient symptom assessment with tables and checkboxes. Save it once, insert it into any note, and voice edit it on the spot.

💾 Step 1: Save a Form as a Quick Phrase

  1. Click on the + icon.
  2. Delete all the instruction text so the box is empty.
  3. Paste the form you want, including tables, checkboxes, or structured text.
  4. Select everything and click the floppy disk icon to save it as a Quick Phrase.

📂 Step 2: Use Your Saved Form

  1. Open the patient file where you want to fill the form.
  2. Click on the + icon.
  3. Delete the instruction text.
  4. Click inside the empty text box to open the icon pop-up.
  5. Click on the Saved Quick Phrases icon.
  6. Select your saved form from the list and insert it.

🎙️ Step 3: Fill the Form with Voice

  1. Scroll down until you see the Voice Edit button.
  2. Start dictating your edits: fill checkboxes, add patient names, enter details.
  3. The system will update the form live with your changes.
