Below is a step-by-step guide to help you add questionnaire results (like MSQ, PHQ-9, or others) into your patient’s file using a custom prompt template.
🧠 Step 1: Create Questionnaire Specific Template
1. Go to "Create Template"
From the left sidebar, click on Create Template.
2. Click "Create Custom Prompt"
This lets you write your own prompt for analyzing questionnaires.
3. Add Title and Prompt Instructions
Prompt Example:
✅ Your template has been saved successfully.
👤 Step 2: Analyze your patient's results
When your patient has submitted a questionnaire:
- 👤 Open their Profile

📋 Paste Their Answers.

📌 Choose Your New Template

🚀 Click Submit
🎉 Great! You’ve successfully summarized your patient’s questionnaire and generated a clean note.
Now it is time to meet the patient ▶️
📝 Step 3: Add the Questionnaire to the Session
Before you start recording, click Add Note as Context.
This will attach the questionnaire summary to your session.
🎙️ Step 4: Record Your Session
Click Record, then select your usual template.
The session note will include both your new input and the questionnaire results.
🙈 Oops! Forgot to add as Context?
No worries! You can simply copy the summary and paste it into the current note manually.
🔧 Did the questionnaire results attach in the wrong field? We can fix that!
To edit your template:
1. Go again to Create Templates.
2. Scroll to your saved templates and click the ✏️ Edit icon on the template you use.
3. Navigate to the section you want (e.g., Objective) and in the Special Instructions field, write: “Include questionnaire results and analysis here.”
🎉 Congrats! You have added questionnaire results and included them exactly where they should be.
