Below is the guide on how to add information from previous visits in your new note, whether it’s a follow-up visit or the patient’s condition has changed.
Classic Method: Add Note as Context
You can pull information from a previous note into your current one.
This is useful if a colleague has already triaged the patient or if your patient has responded to a questionnaire and you want that data in your new patient note.
To do this,
- Go to the patient’s thread.
- Find the relevant past note in the context manager on the left side bar.
- Check the tickmark that note’s details into your new note.
To set this as the default
1. Go to Settings
2. Enable “Append last visit or AI-nurse intake as context”
This will always include the most recent note automatically.
If you forget to add context before the meeting, click the merge icon in the top-right to combine your note with the previous one.
To do so, click on the merge icon in the top-right corner of your note.
Just remember that Add note as context should be clicked before starting the meeting, while the merge option can be used afterwards if you need to include additional details.
Precision SOAP Note
You can add to an old note and create a brand-new one with key details from past visits already inserted and new details added, all while maintaining your familiar template.
Here’s how it works:
Step 1: Click the Add Prechart button ➕
✂️ Step 2: Press Ctrl +A
(Windows) or ⌘ +A
(Mac) to select all, then hit Delete
to clear all unwanted pre-populated text.
📋 Step 3: Copy the entire previous note or just the sections you need (history, exam, chat transcript, lab results) and paste it into the field.
🎙️ Step 4: Click the blue Precision Voice Edit button.
You can have your entire session with the patient here or simply dictate your updates—e.g., “Order T4, T3, and TSH labs on 2025-05-16.”
🎉 Finally, a pop-up will display the original note side-by-side with your new data slotted into the correct SOAP fields. Review the changes, then Approve, Reject, or refine them as needed.
🎉 You have successfully combined old and new information into a comprehensive patient note
